1. Homeless and/or No Phone
   • Patients who are homeless or do not have access to a phone for communication.
2. Severe Mental Illness
   • Patients with severe mental health disorders that would interfere with their ability to comply with the RPM program.
3. Uncontrolled Substance Abuse
   • Patients with active and uncontrolled substance abuse issues.
4. Severe Communication Barriers
   • Patients with language or communication barriers that cannot be accommodated by the program’s resources.
5. Non-Compliance History
   • Patients with a documented history of non-compliance with medical treatments or monitoring protocols.
